Pre-placement documents

In addition to any Police Check, Working with Children Check and Immunisation documents, you may be required to complete and return other forms before you can start placement.

Prior to completing any health placement, you must complete and submit the online InPlace form: Consent and Acknowledgement. This online form outlines how we record your consent to share your personal information with placement providers, and your acknowledgement of your responsibilities for attending placements. It also includes important points about your placements that you need to understand and agree to.

Please note: If you are under the age of 18, a different process will apply. Please contact the Placement Partnering & Operations team by submitting an enquiry via WBL Placement Enquiry Form, and you will be provided with further information.

Student Undertaking Online Form

The Student Undertaking is a requirement of the Department of Health that needs to be completed by students in addition to the standardised placement agreement that La Trobe has with your placement provider. All students completing a health placement will need to complete and submit the online InPlace form: Student Undertaking.

The Student Undertaking is a legal requirement of students that must be submitted prior to and adhered to during all clinical placements.

If you wish, you are able to seek legal advice before signing the agreement. You can access free and confidential legal help via the La Trobe Student Legal Service.

Child Safety Standards

If you are enrolled in a course that includes placement at an internal (La Trobe University) clinic you are required to complete the University’s Child Safe Standards Training on LMS [La Trobe log in required]. This training includes two modules and a quiz. You will receive a certificate on successful completion of the quiz, which will need to be uploaded to InPlace.

The La Trobe Child Safety Standards are based on the Standards included in the Child Safe and Wellbeing Act 2005 and Victorian Child Safe Standards, and aim to strengthen laws to protect children from sexual abuse and exposure to sexual offenders. More information in our statement on Child Safety. You will be notified by the Placements Team if you are required to complete this training.

National Disability Insurance Scheme (NDIS) Worker Screening Check

The NDIS Worker Screening Check is an Australian-wide safety check. It is a way to ensure that key personnel and workers in certain types of roles do not pose an unacceptable risk to the safety and wellbeing of people with a disability.

Registered NDIS providers need to identify roles in their organisation which are considered to be risk-assessed roles.  Any student engaged in a risk-assessed role is required to have a cleared NDIS Worker Screening Check prior to commencing placement with an NDIS provider.

You can apply for an NDIS check by completing the online application form via the Service Victoria website. You will be required to provide evidence of your identity as part of the application process. If you unable to meet the identity requirements, you will need to contact Service Victoria directly for guidance on what other documentation you may be able to provide.

The cost of an NDIS Check is $128.20 and it is valid for five years. There is no fee waiver available for students.  The estimated processing time for an NDIS Check is three weeks. More details on the check can be found here.

During the process of applying, you will be asked to nominate your employer. La Trobe University will be your nominated employer for the purpose of this check, and our registration ID is 4-433C-567.

Students MUST obtain a Full Clearance (paid); a Volunteer Clearance cannot be accepted.


Once you have completed all required documents, you will need to upload them to InPlace for the Placements Team to check  (La Trobe login required).
